Form "Date" type, format option with "single_text" widget

We should add more details to the docs on what formats to use when you're using the single_text widget type.

Specifically, the formats should be derived from: http://userguide.icu-project.org/formatparse/datetime (scroll to Date/Time Format Syntax). A few common examples would also be great.
