[Book][Doctrine] All URLs to doctrine-project.org are outdated.

Symfony standard edition comes with doctrine 2.1 but the following URLs still point to old version 2.0 docs:

We should always point users to latest version of documentation, hence. I think its a good idea to update all the above v2.0 links with correct v2.1links.

yeah, we indeed need to link to the 2.1 doc as stable Symfony releases have never been compatible with Doctrine 2.0
