Changing the new issue template to the form format.
Please review if all the fields marked as required are really required.

GitHub Documentation: Creating issue forms

Description

• Added new yml files for bug, feature request, guidance and documentation
• Removed the old markdown templates
• config.yml - Disabled the option to choose a blank template
• stale issue action: Removed the recently added feature-request and bug labels of the list of exempt-issue-labels, as it impacts the response-requested and closing-soon cycle. Will revisit this change later on.
