Maintenance: Add new Discord invite

We're moving away from Slack and onto Discord. We should update our main README and Docs homepage (docs/index.md) to inform customers how to join the community: https://discord.gg/B8zZKbbyET.

Why is this needed?

AWS Developers Slack channel lambda-powertools will be archived in favour of our new home on Discord.
