Add CODE_OF_CONDUCT file that links to the actual CoC

The `.github` approach isn't always super visible, so having a
`CODE_OF_CONDUCT.md` file in this repo will help. Linking rather
than copying content to keep things maintainable in case of changes
to the CoC.
